Instructions For Liability Waiver Submission
In order to reserve your spot on the tour we need a fully completed form submitted to us. There are 3 ways to submit the form.
- Print the form. Fill it out completely and mail it to us at the address on the waiver. Once received you will receive an email confirmation that your waiver has been received.
- Print the form. Fill it out completely. Take a clear, high resolution photograph of the form and email it to us at full size. You will receive a confirmation email that your waiver has been received.
- Digitally sign the form using Adobe Acrobat. You must have a Digital ID for this process. More information on how to create and manage your digital signatures can be found HERE. After successfully signing the document please email the waiver.
